HOW THE PRACTICE OPERATES

REGISTRATION
Patients wishing to register with the practice should enquire at reception. Registered patients aged 16-74 who have not been seen for three years are invited to see the Practice Nurse for a routine health check. We also offer routine health checks to patients aged over 75 who have not been seen in the previous 12 months.

HOW TO SEE YOUR DOCTOR
The Health Centre Smithy Road, Smithy Road, Coedpoeth
We run an OPEN surgery every morning Monday to Friday. Patients will be asked their preference of doctor when they book in at reception. Evening surgery by appointment only on MON/WED/FRI.

In the event of being unable to keep an appointment, patients are requested to advise reception at the earliest opportunity so that the appointment may be offered to another patient.

REPEAT PRESCRIPTIONS
For convenience please send or hand in your Repeat Prescription Slip to the receptionist at least 24 hours before you require the prescription. Alternatively for Coedpoeth patients both pharmacies do a daily drop off and collect service as do Boots in Regent Street, Wrexham or they can be faxed to the surgery.

The doctors would like to see all patients on repeat prescriptions at least once every three months.

Disabled access is available. Please let us know if you require any assistance or an appointment at a more convenient time and we will endeavour to provide this at a mutually convenient time.

HOME VISITS
Is a service we offer only to patients who are housebound or immobile. If you are incapacitated or too ill to come to the surgery you can request a home visit. You should telephone before 11am so that your request can be triaged in order to assess the necessity for a home visit. Please remember that the doctor can see several patients at the surgery in the time it takes to make a house call.

Senior citizens who are unable to attend surgery can be visited in their own home

HOLIDAYS & SICKNESS
If one of the Doctors is on holiday or is sick the other partner will look after the practice. If both Doctors are away from the practice, a locum Doctor will be on duty.

ZERO TOLERANCE POLICY
The Practice operates a zero tolerance policy whereby any patients who are violent or abusive to any member of the practice team will be removed from the practice list.

IMPROVING OUR SERVICE
We are eager to improve our standard of service. If you have any suggestions please let our receptionist/ practice manager know. In accordance with NHS guidelines we also have an Annual Patient Survey in which you can also make any recommendations.
